Appointment Information

Our waiting room

We accept most major insurances and Medicare. We encourage all patients to confirm their insurance coverage prior to making an appointment with our practice. Please bring your insurance card, identification, and referral (if necessary) to your appointment. We also request that you arrive early to your first appointment to complete a medical history; please bring a list of all current medications and any lab results or x-rays you have to this appointment. Please also bring your insurance card and a photo id.

On your initial visit please be prepared to share your medical and surgical history with the doctor, this would include a list of the medications you are currently taking (prescription and over the counter.)

Bring any lab or X-ray reports you may have had done that are related to the condition you are coming in for. Our office can assist you in obtaining this information if you notify them of the date and location where the test was performed.
